Beginning the 2013-2014 school year, San Jose Unified School District is requiring all middle school students have an ATHLETICS PARTICIPATION APPLICATION FORM be completed by your physician and turned in to Student Services prior to tryouts or practices (for a non-tryout sport). If you plan to participate in a sport during the 2013-2014 school year, be sure to schedule a physical ahead of time. The Athletic Participation Application form is to be completed once per school year and is available in Student Services or on the Bret Harte Web site.
Student Portal Access: (students will see only their data)
• *Login is Student ID
• If students logged in last year, they will use the password they setup.
• If they are new to IC their Password is «first name initial»«last name initial»birthdate MMDDYY» (they can change their password after logging in).
Parents who forget their Portal log-in or password can click on “Problems logging in?”, type in their email address and they will be sent an email with their login information.
• If an email address is used for more than one person in IC this feature will not work.
