Use of School Facilities

In order to request the use of a school facility it will be necessary to complete an Application for use of School Facilities. You will need to download and print the application along with the rules and regulations (PDF), complete the form fully, sign it as well as the rules and regulations page and then return all 3 pages to the school you are requesting.

If the date(s) requested is available the on site administrator will sign the application and forward it to our office. Please contact the Facilities Use Technician at (408) 535-6712 with questions about the fees for use of school facilities and insurance requirements.
