Use of School Facilities

If you would like to request the use of San José Unified facilities for non-district activities, click here to download and print an application.

Please complete the form, sign the form AND the rules and regulations page, then return all three (3) pages to the school site where you are requesting use of facilities. If the school has available facilities on the date(s) you request, the administrator will sign the application and forward it to our office.
