Q. My child has an appointment with their pediatrician next week to get the H1N1 vaccine. I heard San Jose Unified is having a Vaccination Program, should I cancel my doctor’s appointment?
If you have an opportunity to get your child vaccinated by their doctor we recommend you do so. The District Vaccination Program will not begin until our supply of vaccine becomes available.

Q. I can’t find the form on the SJUSD website to give consent for the flu vaccine. Where is it?
A. Go to Quick Links, click on H1N1, then click on Vaccination Information.

Q. Where can I find the vaccine permission slips for those of us who can’t go to the meeting?
A. It is not mandatory you attend the parent information meeting to receive the vaccine permission slip. Every school will be sending home a packet with the student. You may also download the permission forms by going to the website under Quick Links, click on H1N1, and then click on Vaccination Information.

Q. When will children be able to receive a booster mist? My children got the H1N1 vaccine mist two weeks ago but now the doctor is out of vaccine and does not know if they will have any for my kid’s booster. Can my kids get the booster through this SJUSD vaccination program?
A. Children 9 and under need a second dose of the vaccine and must wait 28 days before they receive the second dose. Yes, they can get the second dose through the District Vaccination Program. Your child will need to bring their yellow immunization card that documents when he/she received the first dose. You are urged to continue to check with your doctor’s office if they get more vaccine. At this time, we do not know when the District will receive our vaccine supply.

Q.  Can we only opt for the nasal mist for our child?
A.  The nasal mist is approved for healthy children who are from 2 – 24 years of age.
If your child has chronic health conditions they need to get the injectable (shot form) of the vaccine.

Q. I am trying to confirm that my child can get the nasal vaccination without Mercury. Is this going to be a standard for the school or something I need to request on my consent form that is due next week?
A.  The nasal mist vaccine does not contain thimerosal. The shot form of the vaccine contains a small amount of thimerosal. While some parents may be concerned, the Center for Disease control assures us the amounts are extremely small and not a problem.

Q. I have heard that monkey brains are put in the vaccine. Is this true?
A. No, that is incorrect.

Q. If I turn in the Consent Form late, can my child still receive the vaccine?
A. No. All consent forms must be turned into the health office by Friday, Nov. 13th.
    The only exception will be if you attend a Parent Information Night and fill out the
    Consent form that night (if the Vaccination Day was not prior to the Parent Night).
