Substitute Teacher Requirements


Substitute Teacher Applications will be Available August 1, 2008

Completed San Jose Unified School District Substitute Teacher Application

Please Provide all the Following Documentation

  • Driver’s license
  • Social Security Card
  • Proof of Bachelors Degree
  • (original diploma or official transcripts)
  • CBEST card
  • TB: We will need proof of a current Tuberculosis TEST
  • (within the last 60 days)
  • 30 Day Registered Substitute Permit or Registered Teaching Credential (registered with SCCOE)
  • CSET completion highly desirable

  • NO INCOMPLETE APPLICATIONS WILL BE ACCEPTED
    ALL ORIGINAL DOCUMENTATION MUST BE SUBMITTED WITH THE COMPLETED APPLICATION

    To obtain the 30 Day Substitute Permit or to register a Current Credential:

    Call the Santa Clara County Office of Education to schedule a fingerprinting appointment. Phone numbers: (408) 453-4305 or (408) 453-6828.

    If you need a TB test, contact: Park Alameda Health Facility, 976 Lenzen Ave, #792-5200 or your own doctor.

    Completed applications will be accepted
    Tuesday - Wednesday - Thursday
    9:00AM - 11:00AM and 1:00PM - 3:00PM
