Middle College at San Jose City College2100 Moorpark Avenue, CA 95128 408-298-2181x3996 This San Jose Unified School District and San Jose City College collaborative program was established to provide academically capable “at risk” students with a unique opportunity of completing high school graduation requirements while in a junior college environment. Middle College teachers recruit students from the comprehensive district high schools, interview and screen students for suitability for the program, meet regularly to schedule appropriate junior college course work, monitor course work and grades, in addition to providing the high school core curriculum. All of these activities are part of a comprehensive and ongoing counseling component. The program is monitored by an off-site Principal, a Resource Teacher of Curriculum and Testing, a part time counselor, and currently has three full time certificated teachers in the areas of math/science, English, and social sciences. climate Survey Results (PDF)Home Study ProgramThis program provides instruction/support to families who wish to participate in a more direct manner in the education Web site This program provides the needed instruction/support to families who wish to participate in a more direct manner in the education of their children. The Home Study program is a component of Independent Studies and operates under similar guidelines and requirements. The program is monitored by an off-site Principal, a Resource Teacher of Curriculum and Testing, a teacher coordinator, 1.5 secondary teachers, and one hourly instructor. Necessary Small Continuation High SchoolsThe goal of the Plus Program is to provide social structure and educational interventions, in cooperation with the comprehensive high school, that assists at-risk students to graduate and pursue post-secondary educational and vocational options. School Web site The objective is to provide an educational program where students receive small group or individualized instruction, extensive counseling, and the opportunity to earn additional credits not offered in the comprehensive high schools; each student has an Individualized Learning Plan designed upon entry to the program. In SJUSD, NSCHS or Plus Programs, have been established at all of the comprehensive high schools and serve forty students each. The Plus Programs have a separate faculty consisting of two teachers and a counselor and centralized administration including an off-site Principal and a Resource Teacher of Curriculum and Testing. One program, Career Academy, is housed at O’Connor Hospital and is staffed at 18-1 ratio with the same support administration listed above.

Liberty Middle/High School5845 Allen Avenue, Building 2, CA 95123 408-229-0722 School Web Site The LMHS program allows students the opportunity to receive a middle or high school education through an individualized, project-oriented, independent study program. LMHS is an instructional program of study where students meet individually with a credentialed teacher for at least one hour each week; each student has an Individualized Learning Plan developed upon entry to the program. Students are placed in the program by a Child Welfare Attendance Counselor. The program is monitored by an off-site Principal, a Resource Teacher of Curriculum and Testing, a part-time counselor, and currently has 11.0 FTE, 1.0 FTE serves as a coordinator for the program; in addition, there is 1.0 FTE RSP support staff.
Broadway4825 Speak Lane, CA 95118 408-535-6285 SJUSDs alternative/continuation high school, provides an educational environment for students with multiple risk factors. The school is structured to address students’ personal and academic needs, and students are given the opportunity to learn behaviors that will help them establish a pattern of success. A comprehensive guidance program, flexible scheduling, and varied delivery system emphasize the relationship between school and work. Students all have an Individualized Learning Plan and are placed by a Child Welfare Attendance Counselor. This campus is supported by an on-site Principal, a Counselor, and staff. Julian, CA 95116 408-279-2550 School Web Site These schools provide an academic program which assists students in developing life skills and academic achievement so that they may return to a regular school setting. The middle school and high school programs were established to serve the needs of extremely at-risk youth. Students are referred to the Community Day Schools through Child Welfare Attendance Counselors, Probation Officers, Disciplinary Hearings, Expulsions or suspended Expulsions. The CD Schools are currently staffed with an on-site Principal, an on-site Human Relations Administrator, an on-site Counselor and staffing at 20-1. Each student has an Individualized Learning Plan.
